Is there any possibility to remove this option from the comment box or disable it so it does not appear"?

Is there any posibility to make this utility more usable with showing the form without clicking on text "add a comment"?

Setting the width too low causes the 'message' and 'delete' dialogs to be clipped off. http://bugs.developers.facebook.com/show_bug.cgi?id=4870
